We're seeking a dynamic RN Clinical Manager/Field RN to join our team. What You'll Be Doing:

Lead and Support: Supervise and coordinate clinical services, ensuring high-quality care delivery.
Provide Direct Care: Deliver skilled nursing care to patients in their homes, following physician-prescribed plans.
Manage Referrals and Assignments: Receive patient referrals, assign clinicians, and evaluate patient orders.
Ensure Benefit Coordination: Determine patient home health benefits, medical necessity, and manage insurance approvals.
Personalized Care: Continuously assess patient needs and ensure care is individualized, appropriate, and meets eligibility criteria.
Review and Consult: Review assessments and care plans daily, providing recommendations to clinicians.

What We're Looking For:

Current RN licensure in the state of practice.
Current CPR certification.
Valid driver's license, vehicle insurance, and reliable transportation.
Employment Type: Full-Time
Salary: $ 82,335.00 Per Year
